You can use this form to make a complaint to the Nationwide Health and Disability Advocacy Service about a health or disability service provided to you.

 

The Advocacy Service is a free service that operates independently from all health and disability service providers, government agencies and the Health and Disability Commissioner. Advocates will support you in getting your concerns resolved directly with the provider of the service.

 

After you submit this form an advocate will contact you by phone to discuss your concerns and how you would like to resolve them.
